The accident is simple from what I remember of the details

Please remember the accident happen to sports divers in 15m, these people
very rarely dive with a cage to protect the top of the cylinder.

2 divers, one intact wreck with a long narrow boiler tube in 15m of 
water.
Cylinders on with 1/4 turn back.
Diver 1 enters the tube and crawls/scuttles along its lenght, 8m in (approx)
findes no exit can't turn so starts scullteing back pushing with hands 
ie finns first.

Due to restriction in passage pillar valve was hitting on the roof of
the tube and managed to 'rub itself shut' 3m from end of tunnel.

Diver had no idea of happening only that he could not get a breath
from aqualung.

Diver made it out of tunnel, commenced buddy sharing until problem discovered
and rectified.

In retrospect if the cylinder had of been turned hard on, the initial 
friction of moveing the pillar value would have probably been enough
to stop initial movement.

Hence with a number of use over here as I have already said it is
either

	Full on    or		Full off

There is no half measures in this same, if you see what I mean.

It is an isolated accident, possibility a freak, but its not going
to happen to the people I teach... not if I can help it.

These tunnels are not uncommon in may wrecks around the british coast,
infact it does not even have to be a tunnel just over head plating
or I did hear a unconfirmed report of the same happining to people diving
in thick kelp???
