Dan, This is a total screw-up and unconsciounable. I hope this guy gets raked over the coals completely if what you say is as it happened. It sounds like he's criminally negligent and at the very least, based on the actual events, should NEVER teach again. I did a Mix course a few years back and as you suggest, the instructor DID have us deal with our gear at shallow depths, including all the usual OW steps of removing your mask, dealing with your gear & etc. This girl is dead and the Tech agencies have to deal with it. This can't be defended as a mere "accident," it has to be investigated and all certification standards should be reviewed ASAP. The instructor should be shelved by IANTD until the facts are known and then probably jailed! These days it isn't caves killing people so much as it is bad instruction.
