Tank O-ring failure happens as often as your first stage blowing off the valve, right. That's why you use doubles because the reg would blow by your right or left ear instead of getting the reg square in the back of the head if you use a single tank. PLEASE prove me wrong but I don't see the tank O-ring failure as even part of equipment failure management. I have a couple of 72's that I guess would make the best manifold doubles in the world because they use a pipe tap for the valve(no o ring) and have a lead filled burst disk.
