    Yes, three times that I remember in the last 20 years.

   Once when I was diving in cold (34 degrees) fresh water my main 
regulator froze up (uncontrolable free flow) in 80' of water. When I 
reached the surface on my pony, the first stage and valve on my main 
tank were a solid block of ice.

    Once while diving in the Caribbean, a person who I wasn't diving 
with came to me at about 110' completly out of air. You would be 
amazed out how much air can be consumed by a panicked diver.

     Once while fighting with a lobster my purge button became jammed 
with sand and the regulator (G250) free flowed uncontrolablely.

     Each time my pony allowed me to make a controlled ascent to the 
surface.
