Another failure mode is that the two bladders can get twisted together in the narrow part behind the neck. I wasn't there to see it, but apparently it happened to a fellow in a cave and he had to swim out in a nonhorizontal attitude because only one side would inflate. If the overpressure were on the opposite side than the inflator (no idea, since DiveRite made my wings and they put everything on the left), that sort of thing could keep air from venting.
