Anthony, I agree with you about the way to deal with a frozen inflator. It could be hard though, the house is most probably, as you state, frozen together with the inflator and cant be removed. As I do most of my diving in cold water I use a special house witch is easier to disconnect even if its frozen. One thing to consider is that these things could happen pretty fast. Mostly due to poor use of the inflator, to long filling of the wing. If it happens and you go for the inflator and dont get the house of the inflator and then go for the valves you would already be doing a quit fast ascend. In this case its obvious that the overpressure relief valve did not operate as intended or not at all. The pure size of a bondage wing completely filled and the diver rocketing towards the surface could explain why the diver did not turn the valve off. It could be poor skills to indeed.
