Joel, it was deep air and lack orf redundency - he took a stage of gas and a stage of air - ( no other mix bottle) , had a problem , as did his buddy who was in the same config, and died in a cluster . It don't get any clearer than that. The "analysis" is simple - don't dive deep on air, dive redunend gear in onverhead environments. Next case.
