I believe that 40% O2 was the maximum FO2 used by NOAA for uncleaned
cylinders, and folks just copied that.  Morgan Wells mentioned that at
the '90 ('91?) AAUS Symposium in Hawaii, and also mentioned that NOAA
had been using 50% O2 without cleaning, but didn't admit it.
