George,

Approximately two years ago a diver out here (CA) dove air to 250' and
switched to a 72cf of mix with intentions to break his personal depth
record, he seemed to have used the mix up quite quickly (equipment
inspection showed mix bottle was empty).  We assume he shifted over to
air again on the bottom after using up his mix.  His maximum depth was
311' and his computer showed a total dive time of 11 minutes.

A non-diver on the boat witnessed his arival to the surface due to the
noise as he flew out of the water.  He was diving with a faulty drysuit
valve and had it closed.  The coroner noted that he had contusions to
the back of his head, likely due to a convulsion.

He was a Padi Instrutor and full cave certified.  He had no other
technical training outside of cave and had done a number of dives
exceeding sport limits.

The whole dive was a CF.  His buddies decided not to dive mix and only
wanted to only go to 200' on air.  They followed him to 250' on air,
watched him switch gases and never saw him alive again.  The plan was a
bounce dive for a depth record ... it was both.

I forget his name, but will get it to add to the list.
