While we hear so much about the need to have kit cleaned for
oxygen use, I don't remember ever hearing of any incidents 
which have resulted from using equipment which had not been
cleaned.

Could anyone post details of documented cases where the cause
of the incident could be attributed to using gear which had not
been cleaned, whether during filling or during diving.
