This is only endemic with commercial recreational systems. There are
several areas in the world which rely on instruction by example and
judgement of the individual once a given level of experience is
reached. France and the UK are two that I have experience of. The
majority of training after the basic 'I can dive with someone of
similar experience not an instructor' level is focused on dive leading
and planning experience.
