A funny thing happened while leisure diving in the Carribean. We had just a little deco to do from a short deep dive. At 1000 psi the o ring on the tank k-valve sprang major leak. I shut valve and finished deco on buddy's air. When we got on the boat the divemaster said, "Oh yea, that happens every now and then. The o-ring seats in the k valves are a little too deep and I sometimes have to pop the air on suddenly to get the o-ring to seal. But often when I do that, they will leak when tank pressure goes down to 1000." Notes: (1) I never did like "luxury" dive operations where the divemasters change your gear over. (2) I had never really thought about an o-ring failing because the pressure was too low, but in retrospect, it makes perfect sense.
