Apparently someone had a home made adapter to connect pure O2 bottles to scuba regulators. They connected it up, cracked the valve (as opposed to turning it on slowly) and it all got rather hot so they turned it off. O2 fire. They think he had some brass shavings in the adapter. Probably an extreme case. I know this story is second hand so don't bother pointing it out. By the way, I used to work in a gas station in uni. vacations and the guy who owned it always filled gas tanks, as he did everything else, with a lighted cigarette in his mouth. He is still alive and well. Guy.
