The real expert on getting dive gear on International flights has to be my
partner, John McKenney.  He has flown more shit to more places than anyone
I know.  Perhaps he would give you some info. 
 
I have flown lots of stuff (filmmaking equipment) domestically, and how I
have succeeded in the past is by giving the Sky Cap a $50 bill as soon as
he walks up to me.  All my stuff goes on, with NO excess baggage charges. 
 
That used to work with Intl. flights, but now you have to carry all your
stuff up to the ticket counter - bummer. 
 
The other thing you could try is calling the airlines ahead of time and
telling them that you are a doctor and need to transport some lifesaving
equipment. You are not "techncally" lying - your scuba gear is necessary to
keep you alive, eh?
