How oft Rich, reg failure in the off position would be a tough one to beat - should not happen unless you have upstrem valves. The freeflow loss usually gives you a few breaths even on a low tank - that happened to me in Cayman - and this is when I finally gave up on USD regs, although I did use one yesterday, but it was a reg I keep in the van to flush my drysuit with, not meant for diving, but I took it anyway. It has a Calypso second stage I got in 1974
