You can get it mail order from the Government Printing Office in the
States.  They have a Web site where you can get an order form to fax
them.  The address is:

but you'll have to hunt around a bit for the order form and the order
number.  They have an email address as well (it's on the page), which
might help.

Don't know, but I've heard that shops generally add a pretty big
markup, so even with shipping it may be cheaper from the GPO.
