Subject: Accident Reports

Can anybody tell me where I can get a copy of accident reports concerning
ocean diving, using air as a bottom mix, depth 40 metres (130 feet) plus and
involving planned or unplanned decompression.

I live in Australia so it would be preferred if it could be sent electronically.

The purpose is to create a starting point for an attitude discussion on
deeper diving.

I would greatly appreciate any help offered.
