I use to be a IANTD instructor, and if my memory serves me correct, all I had to do was mail in a copy of the same physical I used for PADI when I first joined, after that, just pay the annual dues so I could get insurance. BTW the physical was done by a local doc in a box who never even ask if I was going to go diving, let alone teach a class.
