For those interested, on Friday I heard a more detailed account of the fatality in S.Wales over the new year. The site has about 30m/100ft of shallow submerged passage, with three airbells. Minimum passage dimensions are about 0.5m/2ft square, with generally poor vis - this is pretty normal conditions, and any diver would expect to meet them after their first few training dives. The victim had about 20 cave dives, and used one 4l, 200bar (about 30cuft) bottle and one 2l, 200bar (about 15cuft) bottle, both full. His diving partner has many years experience, and used 2l bottles. The victim dived in first, to get the best of any vis. He used 50bar (about 7cuft) on the way in, all from the 4l. This strikes me as a lot, though he still had over 80% remaining to exit. Both divers worked on a section of dry passage for a while, and then headed out. The vis was now effectively zero (again, pretty normal). The victim went first, followed by the other diver. In the last section of sump, the surving diver found the line to be very tight and pulled into an undercut. Realising something was wrong, he left the line and followed the wall to try to regain the line. After a few m, finding nothing, he returned to the line and tied in his search reel (actually most probably a form of spool) and tried again. He found the victim almost immediately, within a few metres of the end of the sump. He swam past him and surfaced, tied off his search reel, and returned to pull the victim out. He attempted CPR for 30min, then called cave rescue. Both the victims bottles were empty.
